* If you choose to schedule an appointment under these insurance plans listed above, the appointment will be canceled.
Evergreen Massage & Wellness only submit claims to Primary Insurance (no dual insurances billing service). Patients will have to submit to their secondary insurance for reimbursement.
*Insurance clients are required to have referral or prescription on file to schedule appointments. Please fax Rx order to 425-697-6100 before scheduling an appointment or your appointment will be cancelled.
IMPORTANT NOTE: Pregnant clients please select Prenatal Massage when scheduling, if you scheduled with a therapist that is not listed under Prenatal Massage, you are taking a risk for the appointment to be canceled the day of your session and cancellation fee will be applied. Prenatal massage is a high-risk specialty that only a massage therapist with Prenatal specialty can perform. For any questions, please call our office before scheduling your prenatal massage.
General Policies:
- APPOINTMENT CONFIRMATION: After your appointment is scheduled successfully, you will receive a confirmation email with the time & date of your appointment (if you didn't receive a confirmation email, please call the office to verify)
- APPOINTMENT REMINDERS: You will receive a reminder email or text message from us before your appointment. (we do not have phone call reminder)
- MUST HAVE A VALID CREDIT CARD ON FILE: to schedule an appointment, nothing will be charged unless you 'No Show' or 'Cancel within the 24 hours.' Unfortunately, we will not be able to schedule your appointment without a valid credit card on file.
- INSURANCE CLIENT: If we accept your insurance, this does not mean your plan will cover the treatment. Final payment determination will be made when your insurance company processes your claim. The client is responsible for full payment of the appointment if insurance denies your claim.
- PATIENT RESPONSIBILITY: The patient is expected to pay the insurance co-pay, coinsurance, deductible or out-of-pocket cost at the time of service. We DO NOT SEND out paper bills to collect payment. No exception!
- NON-PAYMENT: In the event that the client failed to make payment at the time of service, you hereby authorize Evergreen Massage & Wellness to either charge the credit card on file or send an invoice to me via email. All overdue bills accrue interest at 3% monthly and may be sent to debt collection if the client failed to make a payment after 30 days.
- ILLNESS: In the event of illness (i.e. a cold, sneezing, coughing, tickle in throat, a fever, the flu, etc.). We ask you to cancel/reschedule your appointment IMMEDIATELY Our therapists reserve the rights to cancel your appointment If you arrive for your appointment with symptoms of an illness.
- CANCELLATION POLICY: If you need to cancel or reschedule your appointment, please do us a courtesy and give us at least 24-Hours notice or the No Show fee of $50 will be charged. This policy is industry standard and strictly enforce to reimburse our therapists' time and income losses.
